A coroner has determined that the death of Second Lt Max George during a training exercise was accidental, attributing it to flawed training and a malfunctioning alarm system. The incident occurred on June 21, 2022, when George was near the open rear door of a Warrior vehicle that was ordered to reverse, resulting in fatal injuries.
The Ministry of Defence acknowledged the findings and stated it has implemented safety improvements, including prohibiting reversing with the rear door open. The coroner noted the alarm had been faulty since 2018 and highlighted the lack of maintenance protocols for the vehicle involved.
